Narrative vs Systematic vs Scoping Review: How to Choose
Choose between a narrative, systematic, or scoping review by matching the review method to your question, evidence base, and intended conclusion.

Choose the review type from the conclusion you need to defend. Use a systematic review for a focused question that requires an exhaustive, reproducible search and critical synthesis; a scoping review to map the range, characteristics, concepts, or gaps in a broader field; and a narrative review to explain, integrate, or critique a topic when interpretive structure matters more than exhaustive study identification.
The labels are not a hierarchy from weak to strong. They describe different research purposes. A carefully argued narrative review can be more useful than a poorly designed systematic review, while a scoping review is not a shortcut for avoiding appraisal or a substitute for an answer about effectiveness.

These are typical patterns, not permission to use vague methods. The title on a manuscript does not determine its rigor; the question, protocol, search, selection, appraisal, and synthesis do.
Start with the decision the review must support
Write the intended final sentence before choosing the method. For example:
- “This review explains how theories of scientific trust have evolved.” This is primarily interpretive and may suit a narrative review.
- “Among adults with insomnia, what are the benefits and harms of digital cognitive behavioral therapy compared with usual care?” This focused effect question points toward a systematic review.
- “Which digital tools, populations, outcomes, and study designs have been used to study insomnia self-management?” This evidence-mapping question points toward a scoping review.
Then ask what evidence would be required to justify that sentence. If the conclusion implies that all eligible studies were considered, the methods must support that claim. If it implies an effect or diagnostic conclusion, study-level validity and synthesis methods matter. If it maps what exists, the categories and boundaries must be designed before the map is drawn.

Choose a narrative review for explanation and interpretation
A narrative review is useful when the task is to connect theories, trace a field’s development, explain mechanisms, compare schools of thought, or give an expert orientation that would be distorted by a narrowly enumerable question.
Its flexibility is both its value and its main risk. Authors decide which evidence anchors the story, so unreported selection can make a persuasive narrative appear more complete than it is. Improve transparency by documenting:
- the review’s purpose and boundaries;
- the databases, reference lists, or expert sources consulted;
- the approximate search period and central concepts;
- why particular evidence types were emphasized;
- how conflicting findings and limitations were handled;
- where the text moves from evidence to the author’s interpretation.
The SANRA paper offers six appraisal prompts for narrative reviews, including the importance and aims of the review, literature search, referencing, scientific reasoning, and presentation of relevant data. SANRA is a critical-appraisal aid, not a reporting guideline or proof that a review is comprehensive.
Do not call a selective search systematic
Searching one database with a few terms does not make a review systematic. The defining issue is whether the methods were designed in advance to identify, select, appraise, and synthesize evidence for a specific question in a reproducible way.
A narrative review can still be explicit about how sources were found. That transparency lets readers understand the argument’s evidential base without claiming exhaustiveness that the method cannot support.
Choose a systematic review for a focused answer
A systematic review is appropriate when a well-defined question can be answered by identifying all evidence that meets explicit criteria and synthesizing it with methods chosen before the results are known.
The question need not concern treatment effects. Systematic methods can address diagnostic accuracy, prognosis, prevalence, qualitative experiences, associations, measurement properties, or other evidence types. The exact tools change with the question, but the core safeguards remain:
- define the question and eligibility criteria;
- write a protocol before screening results;
- search appropriate sources reproducibly;
- document inclusion and exclusion decisions;
- extract and verify data consistently;
- assess credibility with design-appropriate methods;
- synthesize without hiding important differences;
- report deviations, limitations, and uncertainty.

A systematic review does not guarantee a pooled number
Meta-analysis is one possible synthesis method. It is appropriate only when the studies, outcomes, effect measures, and assumptions support a meaningful combined estimate. A systematic review can remain systematic while using a structured synthesis without meta-analysis.

Choose a scoping review to map an evidence field
A scoping review is useful when the objective is to describe the breadth and characteristics of available evidence rather than calculate one answer. Typical purposes include:
- clarifying how a concept is defined;
- cataloging interventions, outcomes, populations, or methods;
- examining how research has been conducted;
- identifying clusters and under-studied areas;
- assessing whether a later systematic review is feasible;
- mapping sources that are too heterogeneous for one focused synthesis.
The JBI Manual chapter on scoping reviews provides current conduct guidance. PRISMA-ScR provides a reporting checklist; it does not replace a protocol or explain every methodological choice.
Breadth must still have boundaries
“Everything about digital health” is not a workable scoping question. Define the population or participants, central concept, context, source types, languages, dates, and exclusions. Broad eligibility increases the need for calibrated screening and a data-charting form that reflects the map you intend to build.
Critical appraisal is not automatically required for every scoping objective, but omitting it restricts what you can conclude. A map of study counts cannot establish that an intervention works or that a gap deserves funding. Do not confuse the review type with the synthesis method
Several terms describe different layers of a project:
- Systematic describes how evidence is identified and handled against a predefined question.
- Narrative synthesis is a way to organize and explain findings in words and tables; it can occur inside a systematic review.
- Meta-analysis statistically combines compatible effect estimates; it can occur inside a systematic review.
- Evidence map is an output that organizes the distribution of evidence; it is often produced by a scoping review.
- Rapid review modifies systematic-review methods to meet a time constraint and should report every shortcut and its implications.
Therefore, “we used narrative synthesis” does not mean “we conducted a narrative review,” and “we made a forest plot” does not prove that a meta-analysis or systematic review was appropriate.
A practical review-type decision tree
Use these questions in order:
1. Is the objective to answer or to map?
If you need a focused answer that will support a decision, begin with a systematic-review design. If you need to characterize what exists, begin with a scoping-review design.
2. Is exhaustive identification necessary for the claim?
If missing eligible evidence could materially change the conclusion, use a reproducible, comprehensive search. A narrative approach is unsuitable for claiming that the totality of evidence supports one answer when the totality was never sought.
3. Does the question require formal appraisal?
Effectiveness, harm, diagnosis, prognosis, and other inferential conclusions require attention to study validity. Use a design-specific risk-of-bias assessment, not a generic paper-quality score.
4. Is the field too heterogeneous for one focused question?
Do not automatically choose a scoping review. First decide whether the heterogeneity is the object you need to map or evidence that the question needs narrowing. A preliminary search can help reveal the difference.
5. Is the primary contribution conceptual?
If the value lies in a new framework, historical account, mechanism, or critical interpretation, a narrative review may be the honest fit. Explain the source-selection process and test the interpretation against contradictory evidence.
Three examples of choosing the right review
Example 1: a treatment decision
Question: Do remote blood-pressure interventions improve systolic blood pressure compared with usual care?
This asks for an effect across a defined population, intervention, comparator, and outcome. A systematic review is the logical design. The protocol should prespecify eligibility, outcomes, time points, risk-of-bias methods, and synthesis plans.
Example 2: an emerging technology landscape
Question: How has generative AI been used during evidence synthesis, and which stages, disciplines, and evaluation methods have been reported?
This asks what exists and how it has been studied. A scoping review can map tool roles, validation approaches, human oversight, and evidence gaps without pretending that heterogeneous evaluations yield one effectiveness estimate.
Example 3: a theoretical integration
Question: How do cognitive-load and information-foraging theories explain clinician interaction with evidence dashboards?
This requires conceptual integration across traditions that may not share comparable designs or outcomes. A transparent narrative review may produce the most useful answer.
Common review-selection mistakes
- Choosing a scoping review only because the question is still vague.
- Calling a review systematic because it includes a PRISMA flow diagram.
- Treating meta-analysis as a required marker of quality.
- Omitting appraisal, then making claims about effectiveness or safety.
- Using “narrative review” to avoid explaining how evidence was selected.
- Registering a protocol after screening has already shaped the criteria.
- Selecting the method a journal seems to prefer instead of the method the question requires.
- Assuming a broader review will be faster or easier.
Final review-type checklist
Before committing resources, confirm that you can complete these sentences:
- The review will help this audience make this decision or map this field.
- The primary question is focused or broad by design, not by accident.
- The conclusion requires interpretation, exhaustive identification, or evidence mapping.
- Eligibility and source types match that conclusion.
- The appraisal plan supports the claims you intend to make.
- The synthesis method is separate from the review label.
- The reporting guideline fits the chosen review type.
- The team, time, and information-specialist support are realistic for the scope.
Once the method fits the purpose, document it before results begin influencing decisions. A defensible review is not the one with the most prestigious label; it is the one whose methods can support its promised conclusion.
